EMPLOYEE KILLED BY FALL OFF ROOF

Investigation abstract

Employee #1 was operating an all-terrain vehicle material handling equipment to carry material from the ramp of a material hoist. Employee #1 backed the all-ter rain vehicle up onto the ramp and was killed when he plunged over the edge of th e roof, falling in excess of 77 feet.
