Event description

EMPLOYEE CRUSHED IN THE CHEST BY A FALLING HORSEHEAD

Investigation abstract

ON SEPTEMBER 12, 1989, AN OILFIELD CREW WAS ASSIGNED TO REMOVE AND REPLACE A BRO ND AND STRUCK EMPLOYEE #1 IN THE CHEST, FATALLY INJURING HIM. KEN SUBMERSIBLE PUMP AT JP#1 OIL WELL. THE HORSEHEAD ON THE LUFKIN PUMPING UNIT, M-228D-213-86, SERIES J, WAS DISMANTLED, THE RODS WERE PULLED OUT, AND THE PUMP WAS REPLACED. THE HORSEHEAD WAS RE-INSTALLED WITHOUT BEING SECURELY BOLTED TO T HE WALKING BEAM. AT APPROXIMATELY 2:50 PM, EMPLOYEE #1 WAS ASSISTING IN THE INST ALLATION OF THE BRIDLE ON THE ROD. A HOIST WAS USED TO RAISE THE POLISH ROD UPWA RD TO MAKE AN ADJUSTMENT FOR THE HANGER. AS THE ROD MOVED UPWARD, THE BOX ON THE POLISH ROD CAUGHT ON THE BOTTOM OF THE UNSECURED HORSEHEAD AND FLIPPED IT OVER AND TO THE SIDE. THE UNBOLTED AND UNLATCHED 570-POUND HORSEHEAD FELL TO THE GROU
