EMPLOYEE INJURED IN A FALL FROM CRANE RAIL
Investigation abstract
At approximately 2:30 p.m. on December 18, 1989, Employee #1 was attempting to u nhook a choker from the crane hook, while standing with one foot on the mid rail of the wire rope guard rail and the knee of his other leg on the top rail. A we lded washer that was separating the wire rope broke free of the weld causing the line to go slack, this caused Employee #1 to fall over the top rail to the grou nd. He sustained the following injuries: two broken wrists, fractured pelvis, an d a bruised arm. Employee #1 was hospitalized.
