Event description
Employee dies after being struck by falling man-basket
Investigation abstract
Employee #1 and three other employees were pulling sheet piles. A personnel man- basket was used to raise an employee up to hook the sheet to the crane. The empl oyee was then set back down on the ground and the man-basket was raised up above the sheet. After the sheet was pulled it was swung around to where it was to be stacked. Employee #1 and another employee were turning the sheets (working unde r the man-basket) when the basket broke. The crane operator yelled but Employee #1 was unable to get out of the basket's way. He was struck on top of the head b y the falling man-basket and sustained severe head and back injuries. He later d ied from his injuries.
