EMPLOYEE INJURED WHEN HAND CAUGHT BETWEEN ROTATING SHAFTS
Investigation abstract
At about 10:25 a.m. on September 20, 1989, Employee #1 was feeling circular smoo immediately. The shafts stopped with employee #1's hand caught and bruising his wrist. Employee #1's injuries did not require hospitalization. th blades rotating on a shaft with his fingers. There was a horizontal and paral lel shaft located approximately 2 in. above shaft with blades 8 in. to 9 in. apa rt. From the back side of the machine where employee #1 was standing, the top sh aft rotates down and toward employee #1 and the bottom horizontal and parallel s haft rotates up and away from employee #1. Employee #1 stated while feeling of t he blade side with his right index finger he twisted his hand and caught his fin gers between the rotating shafts pulling his hand in between the rotating shafts that were 2 in. apart. The paper tube re-cutter operator turned the machine off
