Employee crushed and killed by descending conveyor hoist
Investigation abstract
Employee #1 was working on a Simplimatic Engineering Co. (SECO) depalletizer (mo conveyor descends to the loading position. This is contrary to safe industrial practices. While the line was on emergency shutdown, Employee #1 entered the hoi st chamber without pressing the stop button and was crushed and killed by the de scending conveyor. del 400, serial #5818-586-509), which transfers 2 liter plastic bottles, piled o n pallets in 8 tiers of 110 bottles each, to the filler line. The pallet of bott les enters a hoist chamber and is carried aloft by a conveyor hoist. The hoist c hamber also serves to keep the bottles centered on the conveyors. From time to t ime a bottle will fall off and enter a small hoist sump. The hoist sump is a cha mber containing various gears that is located under the lower position of the ho ist conveyor. One of Employee #1's jobs was to keep this sump clear of bottles. When the final tier of bottles is removed, the hoist doors open before the hoist
