Employee struck and killed by rod projected from press
Investigation abstract
At approximately 2:00 a.m. on March 16, 1990, Employee #1 was trying to loosen a frozen tie rod support cylinder on a Verticast 400-ton die cast press. He place d a steel bar between the two platens and used the power of the press on the cor ner where the tie rod was located to maximize the pressure at this location. The approximately 3 in. diameter steel rod bent slightly and then was projected fro m the press, striking Employee #1 on his head. Employee #1, who was not wearing head protection, lived until April 14, 1990, when he died as a result of the mas sive head injury.
