Employee hospitalized when struck on head by falling I-beam
Investigation abstract
Employee #1, the leadman, and a laborer had erected a three frame high scaffold he basin, the end by the wall bounced in the air and struck Employee #1 on the l eft side of his head. He was hospitalized. (Bil Jax Co.) in an 18 ft 4 in. deep basin. An I-beam was set from a cement wall to an opening in the frame of the top scaffold 2 ft 2 in. from the top on the e xtreme outside leg. A Milwaukee 1 ton electric hoist was swung from the beam to lift a 500 lb trough. The trough was then cut loose and lowered to the floor of the basin. Employee #1 instructed the coworker to lift the 500 lb trough so he c ould swing the hoist east and west. This maneuver caused the scaffold to tip nor th, pulling the unsecured I-beam off the cement wall as it fell. The other end o f the beam pulled out of the scaffold frame.
