Investigation abstract
AN EMPLOYEE WAS DISCONNECTING THE DRIVE MOTOR OF A 480-VOLT EXTRUSION PRESS. THE POWER SOURCE FOR THE MOTOR WAS NOT DEENERGIZED AND LOCKED OUT. THE EMPLOYEE WAS ELECTROCUTED. (THE REPORT DID NOT STATE HOW THE ELECTROCUTION OCCURRED.)
