Event description
Three employees killed, six injured in explosion and fire
Investigation abstract
Employees #1, #2, and #3 were standing on the cover doors trying to remove a pou ring funnel from a mold. They did not know that the mold had separated and was s pewing hot metal in the pit below them. The hot metal super-heated the air in th e pit and caused an explosion, which threw the cover doors up. When the doors ca me back down, they were not aligned with the rails on which they had been set. E mployees #1, #2, and #3 fell into the pit and were killed. The molten metal spla shed over the entire area, burning Employees #4, #5, #6, #8 and #9. Employee #7 sustained broken bones in his leg and ankle.
