EMPLOYEE KILLED IN FALL FROM ELEVATION
Investigation abstract
Employee #1 was removing rock ballast from a roof with a four wheel roof buggy i n preparation of removing the old roof and installing a new one. Employee #1 was assigned to a three man crew and was instructed by the foreman on the jobsite t hat for safety reasons three men must work together at all times. While two men worked, the third man observed. Employee #1 left his crew, loaded a roof buggy a nd went to the chute to make a dump. Employee #1 was found on the ground 40 feet from the chute area by another employee. Employee #1 fell from the roof to the ground and was killed.
