EMPLOYEE KILLED WHEN FELL FROM CHURCH ROOF
Investigation abstract
Employee #1 had been assigned task of putting wire guard over gutter. He ascende loyee #1 died approximately 23 hours later on December 15, 1989. d a roof by ladder on south end of the building moving north to gutter on east s ide of the roof. Two coworkers on scaffold at north end back of a church heard s omething hit the metal gutter, one asked Employee #1 if he was alright and he sa id "yes" out of sight from the two coworkers on scaffold. Shortly thereafter cow orkers on scaffold heard Employee #1 saying, "who" or something similar, and saw him in mid air falling. Employee #1 fell approximately 28 ft from north-east si de of the roof that had approximately 50 degree pitch, landing on front part of his body breaking both wrists, and incurring massive head and body injuries.
