EMPLOYEE DIES AFTER FALLING FROM FORKS OF LIFT TRUCK
Investigation abstract
On November 21, 1989, Employee #1, the operator, was using a high lift rider ord er picker to get a boxed cabinet off a rack shelf. There was not a pallet or flo at on the lift. The shelf was 7.5 feet high. Employee #1 raised the forks to 8 f eet or 9 feet and then stepped over to the rack to get the box. The box was abou t 2 feet by 2.5 feet. The box and cabinet weighed together about 60 lb when the employee was placing box on the forks of the lift. Probably, Employee #1 and the box fell to the floor when Employee #1 was standing on the forks. Employee #1 s ustained head injuries when he struck the floor. Employee #1 died seven days lat er. There were no witnesses to the accident.
