Employee struck and killed by exploding tire and rim
Investigation abstract
Employee #1 was arc welding a metal rim leak (a crack) when the mounted tire, wh uded from truck cage use per 1910.177(a)(1). ich contained some compressed air and, allegedly, Stop-Leak butane/propane gases , exploded from the heat generated by the welding process. The force of the expl osion caused the tire, which had separated from the rim, and possibly the rim, t o strike Employee #1 in the head and face. Employee #1 sustained multiple fractu res and related trauma to his head and neck from which he died. The arc welder w as an Idealasc-250 Lincoln welding machine, model 250, serial #AC-221955, code 5 826, 230-466 V, 70-35 AMPS; the tire was a Motomaster LTR 9.50R 16.5 LT M&S--Tra ction Plus; the rim was a single piece, single wall rim.
