Event description

Employee falls 28 ft to his death

Investigation abstract

At approximately 10:30 a.m. on March 19, 1990, Employee #1 was preparing to lowe r an empty tub from the open side of the second level of a building under constr uction. A Lull rough terrain forklift was lifting a full tub of mortar up at the same time. As Employee #1 attempted to pull his empty tub out of the way, he st epped back and through a 3 ft 3 in. wide by 6 ft 2 in. long opening in the floor behind him. He fell approximately 28 ft and was killed.
