EMPLOYEE FALLS FROM WATER TOWER LEG

Investigation abstract

On September 13, 1989, Employee #1 started doing the demolition project of a wat er tower with an employee. Employee #1 and his employee went up to the tower at the same time. Employee #1 climbed up the bars of one leg of the tower while his employee used the ladder. Employee #1 was carrying a rope used to hoist torch h oses up to the catwalk. Both men were almost at the catwalk when the employee he ard a snap he saw Employee #1 fall to the ground. The employee hurried down and checked to see if Employee #1 was still breathing. Then, the employee called par amedic. Employee #1 died.
