Electrician killed in fall from storage rack
Investigation abstract
Two electricians were re-aligning electrical conductors over the center of a 23 ft high Rack-Master storage rack. Employee #1 was climbing the storage rack's en d frame pieces, horizontal members 30 inches apart, to get to the top of the rac k. When he was approximately 15 ft high, he lost or missed his hand hold and fel l over backward, landing on his head on the concrete floor. Employee #1 suffered a concussion and died of head injuries. A ladder or other safe means of access had not been provided.
