EMPLOYEE FALLS IN UNCOVERED SHAFT AND IS INJURED
Investigation abstract
Employee #1 removed a 48-in. by 41-in. by 0.75-in. piece of plywood which was co vering a 32 in. diameter 80-ft-deep vertical shaft. Employee #1 was going to use the plywood as a concrete splash guard as concrete was poured into a hole. Empl oyee #1 fell into the uncovered shaft and was hospitalized for a fracture.
