Employee killed when run over by highway truck
Investigation abstract
At 1:45 p.m. on July 27, 1989, asphalt was being laid down on an 18 ft wide road e rubber strap used to secure the tarpaulin. The strap broke, and he fell under the first truck's tandem wheels just as it was passing his vehicle. He was kille d. There were rungs mounted into the trailer body on the front end for ascending and descending; the employee did not use this ladder. . One 9 ft lane had been completed. A driver brought a load of asphalt, loosened the tarpaulin over the load, rolled it to the front of the truck, backed the tr uck up to the spreader machine, and raised the truck bed, and the truck was push ed along by the spreader.
