Event description

Employee killed when struck by falling duct work

Investigation abstract

Employee #1 and a coworker had attached a 3/8 in. welded link chain to the top f lange of a 770 lb section of metal duct work. The tie point consisted of the cha in, bolt, nut, and washer. While the employees were positioning the metal duct w ork for bolting in the metal ducts, which were being held by a forklift, exerted excessive strain on the non-flexing tie point. The upper chain link separated a t the weld joint and dropped the metal duct work on Employee #1, killing him.
