FALL - FAILURE OF UTILITY POLE

Investigation abstract

A POWER LINE WORKER WAS INSTALLING A NEW SERVICE DROP FROM AN EXISTING POLE-MOUN TED TRANSFORMER TO A NEW HOUSE. THE EMPLOYEE CLIMBED THE WOOD UTILITY POLE. WHEN HE GOT ABOUT 17 FEET ABOVE THE GROUND, IT BROKE OFF AT GROUND LEVEL, LANDING ON THE TRANSFORMER. THE EMPLOYEE FELL WITH THE POLE, AND IT ALSO LANDED ON HIS LEF T SIDE. THE EMPLOYEE LATER DIED OF INJURIES SUSTAINED IN THE FALL.
