EMPLOYEE KILLED FROM FALL OFF OF A PIPE RACK

Investigation abstract

On January 15, 1990, an employee used a 3-ft-high portable stepladder to gain ac cess to a pipe rack 7.5 feet high. While standing on the rack, he slipped and fe ll as he attempted to fasten his harness to the safety line. His head struck the concrete floor where he received fatal head injuries from the fall.
