Employee struck by a bundle of round bar
Investigation abstract
On December 12, 1989, maintenance man was killed when he was struck in the head out procedures were in effect by a bundle of round bar-stock steel, which was moving along a gravity powered r oller conveyor on which the employee was working. The bundle of steel was loaded onto the conveyor by an overhead yard crane in the steel storage yard. The conv eyor conveyed the steel through the building wall to the shears located inside t he building, the rollers of the conveyor were missing from previous use but the conveyor would still function and was used routinely with the missing rollers. M aintenance man and employee #2 were assigned to remove another roller from the s ame conveyor and put it in the area with three or four rollers missing. No lock
