EMPLOYEE DIES WHILE ATTEMPTING TO CONNECT COIL OF STEEL
Investigation abstract
Employee #1 died because he was struck by a falling material. He was attempting to pick up a coil of steel with an overhead crane operated by a pendant hand con trol. He inserted the inner diameter hole of the coil with the bottom carrying a rm of the "C" hook. The coil apparently did not reach the back of the "C" hook a nd the coil slipped off striking the employee. He was pinned against another coi l.
