EMPLOYEE KILLED WHEN CAUGHT IN ROTATING TRENCHING MACHINE
Investigation abstract
Employee #1 was installing electrical conduit and was struck by and pulled into him into the machine. Employee #1 was killed instantly. a (ladder) rotary trenching machine. Employee #1 was working in a trench next to the building footing. The trench was 35 in. deep by 21 in. wide and 8 ft long. Employee #1 was trying to locate the best place to drill through this footing to run the plastic conduit inside the building. Another sub-contractor was operati ng a (ladder) rotary trenching machine cutting a 6-in.-wide trench at a right an gle from this footing and adjacent to the manhole. When the (ladder) rotary tren ching machine started the trench cut it caught the top of the footing and the op erator lost control. The rotary trenching machine contacted Employee #1 pulling
