Event description

EMPLOYEE KILLED WHEN STRUCK BY FALLING AUGER

Investigation abstract

Employee #1 was crushed to death when a grain auger fell onto him. The Model 410 0 auger was being modified so as to extend an additional 10 feet further into th e grain bin. Employee #1 was lifting the auger off the ground by using a winchin g mechanism which was attached to the truck that carried the auger. At one point he was unable to winch it higher. It was approximately five feet high, which wa s lower than he wanted to go. He went to see what was keeping it from going high er. In doing so, he positioned himself directly underneath the auger. One of the bolts which held the pulley mechanism came out causing the auger to come down, striking and killing him.
