EMPLOYEES KILLED AND HOSPITALIZED DUE TO PROPANE FIRE
Investigation abstract
Two employees were working near a new amine flash unit. The rodded-blind was not removed and an incorrectly marked valve was opened to backwash #4 scrubber. Pre ssurized liquid propane vaporized and escaped from the unit. The vaporized propa ne was ignited by the furnace, burning the two employees. Employee #1 was killed and Employee #2 was hospitalized for multiple severe burns.
