Event description
Employee dies after falling through roof opening
Investigation abstract
At approximately 11:30 a.m. on October 23, 1989, Employee #1 was bent over using an electric screw gun to fasten a sheet of metal decking on a roof. There was a n opening measuring 15 ft by 75 ft where the old roof had been removed. The empl oyee slipped at the edge of the metal sheeting and fell to the inside of the bui lding 30 ft to a concrete floor. Employee #1 suffered multiple fractures and hea d injuries resulting in his death.
