Employee struck and killed by flying relief valve
Investigation abstract
Employee #1 and coworkers were pressurizing a well with water when the relief va lve flew off, striking Employee #1 in the back of the head and killing him. The relief valve was manufactured by Oteco Inc., an equipment company in Houston, TX .
