Employee hospitalized after fall from unsecured ladder
Investigation abstract
Three employees were working on the construction of a downspout on the upper bin level of a new grain elevator. A welder had completed a flange weld and had unt ied the ladder on his way down. The welder was repositioning the ladder to work on a different spout. Employee #1, who was not tied off with his belt, apparentl y stood on the ladder at the same time the welder was descending. When the welde r reached the floor level, the ladder slipped and Employee #1 fell 23 ft and lan ded on his head. Employee #1 required hospitalization.
