EMPLOYEE FELL OFF TRUCK PLATFORM AND WAS STRUCK BY PIPE

Investigation abstract

At 3:30 p.m. on October 24, 1989, Employee #1 was struck by a pipe and fell off the truck's platform. While removing a 5-inch steel pipe elbow to clear a set of concrete from the concrete pump truck piping system, a concrete broke loose hit ting the employee in the chest and face. The force of concrete blew the steel el bow towards employee's chest knocking the employee off the truck's platform. Emp loyee #1 was admitted to the hospital for three rib fractures and a concussion w ith cerebral contusion.
