EMPLOYEE'S THUMB FRACTURED DUE TO GUARD MALFUNCTION
Investigation abstract
On March 6, 1990 Employee #1 was operating a small mechanical power press which was activated by a foot pedal and guarded with a radar proximity guard. The rada r proximity guard was supposed to prevent machine operation if anything was clos e to the point of operation. Employee #1 attempted to remove a piece of work fro m the press. He stated that his foot must have accidentally touched the pedal. T he press was activated because of the apparent malfunction of the radar proximit y guard. Employee #1's right thumb was fractured.
