Event description

Employee injured when caught by belts and pulleys

Investigation abstract

At approximately 7:30 a.m. on June 26, 1989, Employee #1 was applying grease to a grease fitting adjacent to a 16-in. diameter pulley with three 3 1/2-in. V-bel ts that was used to power a Barber Greene asphalt milling machine. His left arm came in contact with the belts and pulley, causing Employee #1 to strike his hea d against the metal guard frame and the asphalt pavement. Employee #1 was hospit alized with injuries to his head and left arm.
