EMPLOYEE KILLED WHEN MATERIALS ON FORK LIFT COLLAPSED

Investigation abstract

Employee # 1 died when a load of fiber materials stacked on a fork lift collapse d and fell on him. There were no witnesses. Fiber board was stacked 3-1 high. Em ployees secured 90 sheets of wood fiber, with each being 4 feet by 8 feet by 0.5 inch. Each insulated sheet weighed between 2,800- 3,000 pounds. The fork lifts are capable of collecting four stacked units. The height of the stack is approxi mately fourteen inches. The total clearance for the stacked material is 42 to 44 inches. The dead employee was found by a co-worker.
