EMPLOYEE KILLED IN FALL FROM A TOWER HOIST
Investigation abstract
At approximately 9:10 a.m. on January 18, 1990, Employee #1 and Employee #2, bot h ironworkers, were dismantling a section of a RP-60 man-hoist. The employees we re working at approximately 140 ft without any type of fall protection. Employee #1 was pulling on a racket tool that apparently slipped from a bolt head causin g him to lose his balance and fall backwards to ground level. Employee #1 died o f multiple injuries.
