Event description
Two employees killed in falls from building
Investigation abstract
Employee #1 was using a 14 ft straight aluminum ladder to climb from the 14th fl oor terrace to the roof. The ladder twisted and Employee #1 fell approximately 1 3 stories to a sidewalk bridge. Employee #2 witnessed the accident and attempted to jump from the 12th floor south terrace to the 12th floor west terrace. He ap parently intended to use a ladder from the west terrace to access an elevator to the ground level. Employee #2 missed the west terrace and fell approximately 12 stories to the ground. Employees #1 and #2 were killed. The straight ladder was not secured to prevent displacement.
