Employee killed when struck by falling plank

Investigation abstract

Employee #1 was standing underneath a scaffold getting tools. A coworker was sta nding on a higher scaffold, 50 ft above him, when a plank gave way and fell, str iking Employee #1 on the head. He was killed.
