EMPLOYEE KILLED IN FALL FROM SCAFFOLD
Investigation abstract
Employee #1 was working on a metal bracket from scaffold erected around a 34 ft ody continued falling and his head hit a metal grating behind him killing him. diameter steel tank being constructed approximately 40 ft above the ground. The platform was 37 in. wide but only two 2 in. by 10 in. boards were used that left a space of 5 or 6 in. on the outside of the planking and about 12 in. between t he inside planking and the tank. This distance varied as much as 4 in. The guard ing was by wire rope around the scaffold and the lower wire rope was 24.5 in. ab out the bracket. Employee #1 had a grinder that fell with him as he fell between the lower wire rope and the outside planking.
