EMPLOYEE KILLED IN FALL FROM DERRICK CROWN

Investigation abstract

Employee #1 was removing crown blocks (pulleys) at the top of a derrick and step ped through the ladder opening falling 30 ft to the monkey board catwalk. The em ployee then fell another 140 ft to rig deck. Employee #1 was fatally injured in the fall.
