Event description

Employee crushed to death by chassis

Investigation abstract

At about 8:00 a.m. on September 19, 1989, Employee #1, of Sealand Service, Inc., was counting the number of chassis in the company's chassis parking area when a coworker pulled out a chassis with a truck tractor. (The chassis are used to pl ace and transport containers.) Another chassis, which was stuck over the one bei ng pulled out, hit a third chassis close to where Employee #1 was standing. Empl oyee #1 was trapped between the two chassis and crushed to death.
