EMPLOYEE KILLED BY A BACK HOE WHILE CHANGING WHEEL

Investigation abstract

Employee #1 was killed when a back hoe ran over him after he and another employe e were attempting to change the right front back hoe wheel due to a flat. The 2, 514 pound, 1975 International back hoe had a Serial Number 234009002201876. Appa rently, the employee planned to start the back hoe to lift the front of the mach ine with the hydraulics when it suddenly began to roll. The machine had been lef t in low range second gear with the throttle completely open.
