EMPLOYEES LOSES TIP OF RIGHT INDEX FINGER-PUNCH PRESS

Investigation abstract

At approximately 8:15 a.m., on August 11, 1988, Employee #1 lost the tip of his right index finger while using a three stage punch press. He was bending eye bol ts on the press. He was placing an eyebolt in second stage. The eyebolt slipped and his right index finger slipped under a press causing the tip to be amputated .
