EMPLOYEE DIES FROM BURNS- PROPANE CYLINDER
Investigation abstract
At approximately 11:52 a.m., on August 22, 1989, Employee #1 died from severe bu rns after a fire broke out while filling a 20-pound propane cylinder. Investigat ors are not sure what started the fire. They believe one possibility is that bot h valves could have been left open.
