Event description

Employee struck and killed by falling lift bar

Investigation abstract

Employee #1 was assisting a coworker in positioning a roll of fabric on a printi ng machine. The lifting bar that was supporting the fabric came loose from the h oist and fell on the back of Employee #1's head. Employee #1 died as a result of the injury. The fact that the fabric lifting bar was not properly secured to th e hoist was the major cause of the accident.
