Employee killed in fall through unguarded floor opening

Investigation abstract

Employee #1 walked down a flight of stairs at the 559 elevation, which was appro ximately 100 ft above grade. When Employee #1 reached the bottom of the stairs, he stepped through an unguarded floor opening and fell to his death.
