Event description

Employee injured in 10 ft fall when scaffold plank tips

Investigation abstract

Employee #1 was removing his tools from a scaffold when he stepped out on a plan k supported by a scaffold bracket that was used for stacking mortar. The plank t ipped and Employee #1 fell approximately 10 ft onto hardened concrete.
