Employee injured when struck by flying metal frame
Investigation abstract
Employee #1 was polishing a 18 1/4 in. long by 6 1/2 in. wide by 2 in. deep meta l frame on a 5 HP Baldor grinder/ buffer, (AT #1458, model 614 85M36, 1500/1800 rpm), when the metal frame was caught by the 16 in. diameter by 2 1/2 in. wide m uslin polishing wheel. The frame spun around the back of the wheel and up over t he top, striking Employee #1 in the forehead. He suffered fractures of the foreh ead and nose, and possible eye damage.
