Employee's skull fractured when caught in rotating mixer
Investigation abstract
At 9:00 a.m. on April 3, 1990, Employee #1, of General Mills, finished replacing a seal on the Cinnamon Toast Crunch drum mixer. He removed his lockout padlock and tag and actuated the mixer to check that the new seal wasn't leaking. He had put his head into the danger zone to observe the seal joint when the handle of the drum's inspection door rotated around and pinned his head against an electri cal conduit that was attached to the frame. He suffered a fractured skull and la cerations. Employee #1 was a well-trained and experienced worker.
