Employee suffers multiple injuries in fall from catwalk
Investigation abstract
At 11:00 a.m. on April 5, 1990, Employee #1 was standing on a catwalk to change the filter on a heating/air conditioning unit. He fell 11 ft 7 in. onto a concre te floor, and suffered a fractured pelvis and right hip, head injuries, a concus sion, lacerations, contusions, and three broken ribs on his right side.
