Employee crushed by crane
Investigation abstract
At approximately 4:42 p.m., on December 7, 1989, employee #1 and 4 other employe over the side of the bridge pulling the crane onto its side where employee #1 w as crushed between the crane and a bridge beam. es were breaking a concrete bridge surface into sections in order to remove the old surface to facilitate raising and widening the bridge over an interstate hig hway. Employee #1 had completed torch cutting the rebar from a section, attached the crane load line to the section, and then moved out of the way to the right side of the crane away from the load being lifted. Crane mfg. By grove, wheel mo unted mod #RT 422, ser # 68617. The crane-bearing surface was not wide enough to extend the crane's outriggers fully. The load being lifted was approximately 10 ,080 lbs. Which overloaded the crane causing it to tilt up then the load shifted
